A caregiver is someone who has a significant responsibility in managing the well-being of a qualifying patient and agrees to assist the patient with the use of medical cannabidiol. A caregiver is designated by a physician while filling out the written certification or recommendation for the qualifying patient. A caregiver can possess no more than thirty-two ounces of cannabidiol oil per patient.
Requirements
- Must be 18 years of age or above.
- Must be registered with the Department of Public Health to obtain medical cannabidiol registry identification card.
- Must have no records of related drug conviction.
Becoming a Caregiver
- A caregiver must submit an application to the department, on a form created by the department, in consultation with the department of transportation, that contains all of the following:
- The primary caregiver’s full name, residence address, date of birth, and telephone number.
- The patient’s full name.
- A copy of the primary caregiver’s valid photo identification.
- Full name, address, and telephone number of the patient’s neurologist.
